I'm being treated in Sydney, and I (and every other FTM I met around there) started on biweekly injections of Sustanon 100. I've never been on a cream or gel.
AFAIK, what T delivery method you start on is generally determined by your treating specialist (usually an endo). Normally they'll have a program they like to follow with their patients. Some prefer you start on gels or cream, some go straight to injections.
My injections cost me about $20 for 3 shots. I have one shot every 2 weeks. That cost does not include seeing the GP to be injected - if you can't find a bulk billing GP (or nurse, or whatever), you may be paying an extra fee for that.
We do have laws (could be state or federal, not sure) regarding filling out your T script. The chemist must retain your script. You must go back to the same chemist for ALL your repeats. You cannot get a repeat within 21 days of having one filled. Each repeat is for enough T for 3 shots.
But they'll explain all that to you in detail when the time comes
