Question about starting with therapy and getting hormones and coming out!

I have no idea where to start from. Do I need to see a therapist first and than does he or she put me on hormones and where can I find one at? I have always felt trapped in a mans body for years and I am ready to come out!

Things will vary a bit depending where you are, but the sequence is apparently pretty much the same, gender therapist, living in role and hormones, then surgery. There is lots of info here at Susans in the Wiki.
