You do have your disability, which is a plus.  But finding a low cost living area that also has the services you need it the trick.

Your area is not know for being very open.  You might want to look to the west coast.  California, Oregon and Washington are good areas.

An hour and a half isn't that bad. Mine is further than that. It amounted to less required visits and that didn't hurt my feelings (or wallet) much at all.

There are also gender therapists who work online, if that would work for you until then.

Did I miss you listing where you lived? I lived in Flint, MI for the past eight years, and had the same struggles. Could not travel out of my county to friendlier Ann Arbor or Detroit due to my disability. I did find a therapist, who ended up writing me a letter so that when I moved back to Dayton, OH I could give it to my new doctor (and now I have started T). It can be very frustrating. I moved for school and added bonus Dayton has a few trans-friendly doctors here.

I do know California, New York, Massachusetts are supposed to be great. Here in the Midwest, I know Ann Arbor is great (but expensive), Columbus is supposed to have a decent GLBT scene, but also Ohio can be kind of conservative depending on the area in which you live.
