I'm usually pretty critical (and always judging against what truly is considered beautiful in a modern day young or older woman) but I'd truly say you may not need much work done, if any. You do have an image that makes up a good portion of the natal females out there who have extra weight and are over 30 years of age. As it is, you seem to have very soft, rosy look and I do see many women on the streets every day who look like this. Now, as said above, loosing weight may hinder or help your look from here - it seems many of the more masculine looking older women out there look much more female with added weight and rosy features. Loosing weight may bring more attention to a bulbous-ish nose or a thick-set jaw line. There are a lot of relative things and for now, I dunno what to suggest apart from trying non-surgical things like weight loss or a longer HRT duration, to see what happens there first.
P.S. I saw your "Queen'd up" image on another thread, and if it helps any, I think you make a super attractive Drag-ish model. You truly look female enough to the point beauty and being female shines through, and I would say both your features and clothing style looks more like a natal woman who is dabbing in Drag culture, rather than you actually being a man. I do think a drag-inspired Queen look truly highlights your appearance though, and works best with your features. Even when not inspired by that culture, things to remember are that, like Drag clothing, lots of accessories, very volumous, layered clothing, and large hairstyles can really soften a look.
