Hi and welcome.  The first step would be to find a good, reputable Gender Therapist to help sort out the questions you will no doubt have.   Once that is underway, then you will have a better idea of your own prioities.
They can also advise on other contacts and support services available.  If you have access to a local Gay/ Lesbian switchboard, they are a good place to look stuff up in your own vicinity.

Main thing is, enjoy being the woman you have always been, but were unable to show to the world.

The first step is very dependant on your circumstances and how you are as a person.  Some people will see a therapist others will get their GP to prescribe hormones straight away.  My first step was telling my parents and girlfriend,  then I went to see my GP and got him to prescribe hormones.  After a few months I tried therapy but found it a complete waste of time.  About 4 months on hormones I went out in girl-mode for the first time, went full time after about a year.

If you have the money and are committed I'd suggest starting electrolysis as early as possible (it takes ages).

All I can say is that my life is pretty plain, I like watchin' the puddles gather rain.
