If I have SRS in Thailand, I don't need a RLT or letter?

I was reading the wiki about RLE. It says this:

"A minimum requirement of 12 months RLE is part of the Harry Benjamin International Gender Dysphoria Association Standards of Care. In many places this differs - for example in the United Kingdom most National Health Service trusts will require 24 months, whereas in Thailand the Benjamin SOC is not usually followed and there is no such mandatory delay."

So, I was going to have it in Thailand anyway due to the fact that it has a great reputation with SRS and because I have family there I can stay with. Does this mean I don't have to worry about RLE crap? ;D I don't even exactly know what RLE is, but sounds like it involves throwing more money at a therapist.

RLE as I understand it is having lived as a your target sex for an extended period of time. Don't know for sure about the general situation in Thailand but I do know Suporn (if I remember correctly) require you have a 12 months RLE and a letter. I had SRS with him but didn't need the letter as I had had FFS with him the year before.

Thai law changed a while ago to require letters before genital surgery, depending on the source, one may have to be from a local psych.

RLE is generally understood to mean living as ones gender identity without reverting to the one assigned at birth out of choice. Legal name change is generally defacto indication of living "full time".

RLE is short for real life experience where you live in the gender role you desire.  I'm not sure but I think you have to documente the starting of your RLE with a therapist.  I think in a lot of cases that one can either go through RLE at least three months  or therapy to get access to hormones depending on your location.

I'm thinking there will be a few there that will require documentation just to cover their own selves to make sure the patient didn't decide this on the spur of the moment and may try to sue them later after regret.

Yes, unless you have FFS with him first. At least that's how it was in my case.

Dr. Preeha in Thailand also requires RLT and a letter.  He will schedule you to see a psych at the hospital before your SRS for your 2nd letter.

I had SRS with Dr Suporn in December, 2010.  I was required to bring a letter and also
have a very brief mental health evaluation the day before surgery.  Here is some info I pulled
from his website in the summer of 2010.

Minimum Psychosocial requirements for Patients Scheduling SRS:
It is not Dr. Suporn's policy to deny SRS surgery to any patient he believes to be medically and psychologically fit to undergo it. As far as is considered reasonable, The Suporn Clinic follows the guidelines of the Harry Benjamin Gender Identity Disorder Standards of Care (HBGIDSOC). Additionally, we always comply with the Medical standards of Thailand. We require patients to provide written evidence that they meet the following criteria:

SRS Patients
ALL patients undergoing SRS MUST provide an original, signed document from a qualified psychiatrist or psychosexual specialist, confirming as a minimum that:
1.   They have been diagnosed with Gender Identity Dysphoria.
2.   SRS is indicated in their case, and is the recommended treatment.
Ideally, the referral should also confirm that:
3.   They have been living full time as a woman for at least 12 months prior to surgery or - if not - gives an explanation as to why that has not been possible in your case. It is accepted that in some countries the "Real Life Test" (RLT) is not a practical possibility.
4.   They have been undertaking a course of female hormone replacement therapy (HRT) unless there are medical reasons (to be indicated) that such a course is not advised in your case.
The letter must show the name, address and telephone number of the signee, for possible verification.
In order to ensure that there are no difficulties with the documentation that you have, please make sure that you scan the letter and send it to me as an email attachment for review, to make sure it meets the necessary requirements. It is essential that you bring the original with you when you come for surgery (or that the original is sent to us direct). It will not be possible to go ahead with the SRS operation without such a suitable letter.
There are no exceptions to this requirement. Any patient who arrives without this evidence will not be permitted to undergo surgery. If you have difficulty with providing such evidence, please contact The Suporn Clinic by email, so we can discuss possible ways of meeting this requirement.
Additionally, if the referral letter from the psychosexual specialist does not specify as in items (3) and (4) above, we require you separately to provide evidence that you:
· Have had treatment with feminizing hormones (HRT) and/or anti-androgens for a period of 12 months prior to surgery, unless there is a medical reason why to do so would not be advised in your case.
· Have lived full time in your acquired gender for a period of at least 12 months prior to surgery, evidenced by at least 2 original documents ( ** ), unless there are clearly given and acceptable reasons why this is not possible or practical in your case, or why a lesser period has been spent prior to surgery.

( ** ) Documentary evidence needs only to be sufficient to show the patient's acquired (female) name, and bear a date at least 12 months prior to surgery. Suitable examples would be - but are not restricted to - passport, driver's license, official ID card, household utility bill, bank statement...etc.

I think that you are missing the entire point of the RLE.

The RLE is a litmus test more for yourself than any therapist. If you can not fit into the life of your target gender it is far better to find out before srs than after.
If you do not desire to involve a therapist any more than you have to then change your name legaly. That is generaly considered proof of going full time. Go full time. That is the best thing you can do for yourself if you are trans. The longer you spend in the wrong body and social gender the longer you are allowing yourself to be damaged by living in what is basicaly hell.

I highly recomend going full time asap and getting into the deep end of the pool. Life is so much better when your outside gender matches the inside gender.

All the surgeons I had consulted with asked me for a letter from 2 psychiatrists. It was quite easy to obtain in Thailand though. I went to Rama 9 hospital and met with 2 different psychiatrists. It took less than 2-3 hours costs me 2,100 baht (about $70). You pretty much can walk in without an appointment. There were 2 other foreign mtf at the waiting area for the same reason as mine. Now, I don't know how long you have to be in transition before the drs will approve though.

Nah, the point of a 1yr+ RLE isn't for our own good: they just pretend that that's the reason. Most of us know before we even start RLE that we are ready for SRS. I think that a more realistic requirement is 1-2 months of RLE.

Can one book with Suporn or Brassard (I care more about the latter) BEFORE one has the necessary psych letters? I'd like to book something for as early as May, but I wouldn't have my letters until as early as April.

I ended up doing the RLT for a year and a half. Part of it was because I moved. Part was financial. And lastly, the surgeon was injured in a horse accident and was laid up for a few months. I'm not sure that my life would have been all that much different had I been able to get surgery any sooner.

As far as knowing where I wanted to go? The RLT didn't change my opinion in any way. However, it was a good thing (for me) to get feedback from some good therapists. They did help me with other life's problems.

There are reasons why many medical professionals require the RLT. It's a good guideline they can measure. It is also a protection for them in that if they follow the standards, they are not at fault if you change your mind.

I've read of a few people who rushed into this, changed their minds, and then spend the rest of their lives griping about it and making it more difficult for the rest of us. I'm sure you're not in that category.

I was a patient of Dr Suporn.  I was able to book my surgery without having my letter.  With Dr Suporn you fill out an intake questionnaire when you book a surgery date.  The questionnaire will ask about RLE, therapy and where you are in the process with your letter.

I imagine Dr Brassard would have a similar intake process.

I have done this already as well, mine is scheduled for December with Dr. Suporn. I booked back in May and said I was working on getting the letter. I had to wait until late June until starting with the psych, and I am hoping like anything I will actually get my letter eventually. I'll start to panic in October if I still don't have my letter.


The proof of RLE documentation for Dr. Suporn seems very simple.
