Finally got the name change, photo change and the 'm' on my passport. Over the moon!!
So my question is: do we have to change anything regarding EHIC card or national insurance number? (in the UK)

May i ask did you get your gender marker chagned?
The NIN i dont think you need to worry about it i did ask but they said not to borther, but dubbled check.
Started Testosterone: 2013
Top surgery: 2014
Bottom surgery: 2016

There are various other things you need to change:
Yes, you will need (or more appropriately,
want
) a new EHIC card. Tell them of your name change by either calling the EHIC enquiries team on 0300 3301350; emailing them at
nhsbsa.ehicenquiries@nhs.net
or writing to EHIC Enquiries, PO Box 1114, Newcastle upon Tyne NE99 2TL providing your full name, UK address, date of birth and, if known, your EHIC personal identification number (PIN).
You need to change your National Insurance & tax details. Your NI number doesn't change; they just associate it with your new name. You can do this online in just a couple of minutes:

This is really handy because at the bottom of the form they'll ask if you want them to update all your records in one go. You'll need your Unique Taxpayer Reference number, which (if you're employed) you can get from your personnel department or accountant. If not, the Job Centre will have it.
Have you applied for a new NHS Number? If not, ask your GP's receptionist. They can't just change your name on their system, they have to issue a brand new NHS Number because each number is linked to a specific gender so they have to write to whoever is issuing these numbers to get a new one.
If you have a driver's licence or own a car, contact the DVLA. You'll need a new driving licence and your vehicle registration needs to go to your new name.
Contact everyone you do business with. Your bank, the local council, your gym, your employers, TV Licensing... everyone. I started by ringing them up & telling them I've changed my name; how do I need to notify them? Some asked for certified copies of my Deed Poll, some were happy with a photocopy, but others just changed it with my phone call.
Don't forget the little things like a Tesco Clubcard. Go through your wallet & pull out everything with your old name on. Contact all of those companies & get a new card issued.

yeah i got my marker changed by providing the name change and a doctor's letter. thanks for the information! so many things i havent even thought about! sounds like i have a lot to do then!

No worries!
If it helps you to know this, I made a spreadsheet to keep track of everyone I'd contacted. I was surprised to discover that I have regular dealings with over 30 different organisations and a couple of them made me jump through a few hoops, so it was essential that I made notes of where I was with everyone.
