Getting PCOS treated now will not affect HRT later. PCOS tends to give you elevated testosterone, which sort of gives you your own HRT.
As to doctors, starting with a therapist, especially if you live in a bias area, is probably a good start. They can often refer you to doctors and will help you deal with all the ups and downs that come with transition. They can write letters making it easier to get HRT and possible to get surgery.
There are multiple surgeries possible. I'm not sure if you mean top or bottom surgery or if you have looked into the possibilities of each. This would be a good thing for you to do and again a therapist can help you with that.
