Cosmetic reasons or to help trans women, were never on our radar, when we did the development of the breast devices. the same was behind the penile implants that were developed.
I lived most of my life as a cis male (so I was told), but in my entire circle of adult males, the bulge in ones underwear was never of any concern.
It may be that performers do things like enhancing it, but most males do not belong to that group, and live a pretty normal masculine life, without demonstrating what they have inside their underwear.
It was a big thing as teenagers when wearing swimming trunk (speedos were the name of the game in those days), same was hair on the chest. I never had either, a big bulge nor hair on my chest, but I was still fully accepted as a guy. Guys do not judge each other by the appearance of secondary sex characteristics, but by the way they do the male bonding stuff!

There are plenty of cis men with small packages and they feel so bad about it that they avoid public toilets and locker rooms. It often stops them from having sex because they dare not show it to a partner.
You say you were raised as a cis male but I wonder how masculine you are considering your comments here such as preferring to present as female.
You see I have always been very masculine and I know that men compare their sizes and all kind of other much sillier than that. I have witnessed it and I have discussed it with a number of guys. I have discussed packages with a number of gays too, You see I have the same attraction to men that gays have because I'm gay myself.
One of the main complaints that brings men to psychiatrists or therapists are difficulty with their penis, Size, erection, libido, etc.
I have a different issue which means I have been with a general psychiatrist for the last 4 years and he is very happy with my transition.
I work in finance and this is a very prominent private psychiatrist in London city.
He has patients that trade millions and run leading law firms and investment banks.
Although he doesn't handle my transition he supports it and asks me about it.
I explained my thoughts on this and he doesn't think it is even remotely silly because the penis is so central to masculinity.
I also have a male therapist who has also discussed this with me back and forth. He found it very important that I formed a position on this.
Then again I have a male mentor who has been going through this all with me and discussed it man to man and he is frankly surprised how little importance trans men seem to put into this part compared to that of cis males.
The moment Alfa men start to bully other men they go for penis size, hair, voice and feminine movements.
They have no issues with showing a bulge, not one bit.
The absence of a bulge though feminises a man so that he isn't really taken seriously.
They put a lot of effort and money into looking as masculine as possible.
You don't need to spend many days on the trading floor or with tradesmen,(carpenters, plummers etc) for that matter to hear the way they talk.
Perhaps those implants were intended for mending after accidents but the majority of implants goes to enlarge existing healthy breasts and penises.
Besides if all these cis people don't care about their sizes then loosing a bit to illness or accidents won't matter to them either. Lol
