My Orchiectomy with Dr. Arnkoff (Southfield, MI)

Hello, my name is Ashley. I am a 19 year old mtf transgender girl, and I am here to post my review after having had my orchiectomy via Dr. Arnkoff in Southfield, (a suburb of Detroit) Michigan.

He has, from every account I have seen, been very well reviewed (none of the "horror stories" like I saw with some Doctors whom actually charged much more, when I was initially searching), which is why I chose him, but I figured one more positive review wouldn't hurt, and I honestly feel he is owed that much from me. To start off, for anyone who is interested in having an orchiectomy procedure done, I would highly recommend him to perform the surgery.

His staff, his office, his technique, his manner, and his professionalism all shined through and made it a very enjoyable experience. A certain member of his staff, in particular, went extremely out of their way to ensure that I had everything I needed in order to have a comfortable aftercare experience. (My surgery was July 31st, I went home on August 2nd.) Also, while the surgery was being completed, I do not think I would have been able to get through it nearly as comfortably as I did, if not for his nurse, Sarah, being there to hold my hand and discuss non-surgery-related topics with me all the while. He also played Glen Campbell via his iPod throughout the entire procedure, specifically at my request as I wanted something to distract me from the sounds of the procedure being carried out, which was oddly comforting as well. (As you can probably deduce, a local anesthetic was used; I'm unsure whether he has an alternative option, but I'm relatively squeamish, and I made it through.) There was literally no request I made, that went unfulfilled. And yeah, I can be a little high-maintenance at times; just ask my boyfriend.

For those interested in cost, the surgery itself was $2,300, while the airfare for a roundtrip via insanelycheaphotels.com, came to about $500. I was VERY fortunate to find an absolutely incredible hotel actually IN Southfield, named "Hawthorne Suites", which was $60/night, for, get this - Wifi, an absurd amount of cable channels for that price including Showtime, a kitchenette (actually, quite literally an ENTIRE kitchen, WITH cups, bowls, silverware, etc.), a fireplace, a spacious washroom, and the room itself was exceedingly clean. I'm still blown away by how inexpensive my stay there was. It was literally more like a well-furnished/appointed condo than a hotel.

Anyways, at the end of the procedure, he was kind enough (although I suppose they were sort of my "property", hehe) to encase my newly-evicted testosterone-producing dwellers in an airtight jar to take with me to my hotel; which may sound somewhat unnecessary, but it provided me an IMMENSE amount of confidence and satisfaction to be able to look at them anytime I wanted to remind myself that I did indeed finally accomplish this goal; and will not be tormented (a bit melodramatic, but true) by them anymore.

As far as the healing process is concerned, so far things seem to be going well. I still have some residual pain and discomfort, but it seems to be dissipating more and more with every passing day. I know that I was a bit of a difficult and perhaps slightly uncommon case for him, as I had had such shrinkage in that area from the prior hormones alone, and my testicles had essentially migrated unusually high into my abdomen. It was noticeable to the extent that at one point during the surgery, he actually joked "ummm... are you sure you have TWO of these?" That's another thing I found very appealing about him; his sense of humor. A bit dry and caustic, but nonetheless, endearing. Sort of like if Bea Arthur were a male urologist.

As far as Misc. information is concerned - I did not discontinue hormone use either BEFORE or IMMEDIATELY FOLLOWING the surgery. Purely as a matter of individual preference, I intend to continue my pre-op hormone dosages, including the use of anti-androgens (with the exception of Androcur/Siterone; I believe they targeted that "specific" area which is no longer there) and I have had no problem with that thus far. I was given two prescriptions post-surgery; one for Vicodin, one for an antibiotic which is meant to be taken 4x/day. They were each called in to the Walgreens pharmacy; very conveniently located directly across the street (or perhaps a block away) from his office.

If I remember anything else worth mentioning, I will certainly post it here. Once again, I would HIGHLY recommend Dr. Arnkoff to perform the Orchiectomy of anyone who is interested in having it done. His website is www.myorchie.com. In summary, I would recommend him on a basis of professionalism of both himself and his staff, technique, aftercare, as well as cost. I truly do appreciate what he did for me; as well as how efficiently he did it. Also, and I know this will be huge for some people as it was for me, he requires NO letters whatsoever in order to perform this surgery. He is of the mentality that it is an individual's choice what they do with said individual's body. As a girl who has always taken matters into her own hands to get what she wants/needs, I wish more people in the transgender medical community were of that school of thought.

As a side-note, for any of those who have successfully gone through this surgery regardless of surgeon, anything positive to report as far as experiences post-op? Feminizing effects, etc. I know a lot of girls have said they feminized the most of their entire transition after Orchiectomy or SRS. I would love to hear any anecdotes! :)

Mine was done by Dr. Nguyen here in Portland.  It was a lot cheaper, $1500, and I recouped at home.  And I had an audience.

There were two medical students there, both women, and he had an intern training.     The medical students asked me about my transition and how I was doing.

It helped to pass the time and about 45 minutes later, I was ready to go home.  I had no complications at all.  And he cleared me 10 days later.

Mine took about 45 mins as well. :)

I, too, am currently located in Oregon (I have family here), but despite the price (and distance) difference, I still decided to go with Dr. Arnkoff. There's something to be said for not having to jump through someone else's hoops to do what you wish with your own life and body. I found that very refreshing.

I have been wantin to get mine done for some time. I asked my GP, and even though he was very sympathetic he said he didn't know anyone in the area that would do it.

A local girl I know bypassed the entire system here and flew to the US for her Orchi under informed consent (not available here) a couple of months ago.
Apart from Detroit being like Kabul or Aleppo, (she stayed in 12 mile, 4 miles worse than Eminem's 8 Mile apparently!) it all went really well and she was able to go across to see relatives in Canada for a holidays a couple of days afterwards.

I feel great after the surgery. The pain has finally begun to REALLY dissipate. Some say there will be a spike in testosterone for a very short period immediately following one's Orchiectomy or SRS, but I feel I have seen some pretty wonderful changes already! My facial features, which were considerably feminine before, seem to have smoothed out even further. The rate of growth of hair on both my legs and my face in particular has noticeably decreased. Also, fat re-distributions are DEFINITELY occurring; I'm a very "slight" framed girl, but my boyfriend has said my body has gotten a really fantastic curve to it since the procedure; also, my upper-body seems to be gradually decreasing in size, which despite my small frame, is a very welcomed change! I have also noticed a fair amount of breast growth, which I know is common for girls after removing their testosterone-producing faculties, but this could also be due to the fact that I have begun taking progesterone for the first time as of the 10th. :)

Any competent Urologist who does them for testicular cancer should be able to do it. That is if they will agree, no accounting for transphobes in the Medical profession either.  I would guess you would  need the usual two letters though.

Here in Mexico the prices are around that, I might get mine as soon as possible as long as the surgeon doesn't gets picky about it.

Great to hear it all went well.  I get a kick out of the fact that his office is 8 miles from my home. I will definately consider him if I have this procedure done. 

I always wonder why take many years off testoterone blockers when you can remove the source in a 45 minute operation.

Hi hun! I'm glad you've decided upon Dr. Arnkoff to perform your surgery. Like I mentioned in my post, he's a great choice. :)

I'm sorry you are having such a conundrum with getting things scheduled. I will try to address your concerns the best that I possibly can. First of all, you should have no trouble whatsoever getting an appointment set up with him. I know there are specific days of the week in which he prefers to perform surgery, but generally he will be available pretty much any week you need him; as per my experience. As I'm sure you're aware, the number on his website (www.myorchie.com) is that of his personal cell phone, so you shouldn't have too much trouble simply getting a hold of him to talk about dates, either.

As far as him addressing SRS is concerned, he did not overtly address it, but I didn't find that unusual, either. He does not remove pertinent scrotal skin during the surgery, the incision in my case was actually very minimal and I do not expect it to interfere with my later decision to undergo SRS. His technique is actually quite ideal, in my opinion as well as given the research I have done, for anyone who does wish to pursue SRS at a later date.

As I mentioned in my post, I had my surgery done in the afternoon on the 31st of July. I left the morning of the 2nd of August. During this time, while there was still soreness, I indeed managed to walk to meet my cab at the other end of the hotel to head to the airport, I managed to go through the whole process that was integral at the airport to get back to where I live, which included a 2 hour, 45 min layover and was about 8/9 hours of travel in full. It wasn't ideal, nor totally comfortable. I definitely felt that to have had an extra day would have been more conducive, but I made it. I'd say if you manage your trip for at least a day longer than I did, you should be recouped enough to do what needs to be done.

As far as Vicodin before the operation... No, but both during and for the first 2 hours or so after the operation will actually be the MOST comfortable, pain-free time for you, as you will still be completely numb from the local anesthetic that is used. Just make sure you pick up the prescription immediately after the procedure while heading to your hotel, etc., because it will definitely come in handy later. He will also instruct you to ice very regularly, which is tremendously helpful with both pain and swelling and should be taken seriously.

I still have his phone # obviously and he has been tremendously helpful every time I have been in need of anything. Because I am so far away though (out of state, out of timezone even), there is only so much he can obviously do if I were to have any complication, which now 2 weeks out from my surgery, I have not encountered in the least.

Definitely consider packing some panty-liners for any drainage post-surgery, and he will also ask that you bring an athletic supporter which fits snugly on your body to ensure that his work stays "put" and heals accordingly. I found this very integral and helpful as well, despite the lack of enthusiasm for the particular garment in itself.

I hope I have helped... if you respond, either at my personal email or back again here, I would be more than happy to give you specific details (cab, hotel, etc.) that worked very well for me and made my experience a matter of ease and cost-effectiveness.

If you don't mind... when you go into your appointment, please say that Ashley from Oregon helped you make your decision. I just would like him to know by my recommendation how much I appreciate what he did for me, and the amount of expertise with which he did it.

I wish you the best of luck! I cannot begin to state how pleased I am with this decision. I do not think I could have been happier or better-treated with any other surgeon. :)

Hello Ashley et al,

Your messages are inspiring...but I'm wondering if I'm "late-to-the-game" with this email, nearly a year after the original posting.

I live in Michigan, so Dr. Arnkoff is close in proximity.  I am considering the Papillon Center in PA for the SRS.

It is good to know that Dr. Arnkoff understands this is a 'first step'...and that things may be needed for future SRS.

This summer is the summer of my orchiectomy.
And it's great to see there's such a good doctor near me.

Question ... Perhaps I am ignorant of the latest methods during transition because I had SRS 28 years ago ... Why an orchiectomy and not a full SRS?  I understand the need to remove the testosterone producing elements, but does not an orchiectomy still leave you with a penis?  Also, it is my understanding that the scrotum is required during SRS, so do I assume that the orchiectomy still leaves you with the scrotum skin?

I apologize for being ignorant in this area ... As I said, my SRS was a long time ago.
