Personally (after watching that particular scene on YT) I felt it was much more satire directed at people who are ignorant of TG people; both Brian and Stewie had stereotypical anti-trans responses...and it appears from the dialogue that Ida (the woman in question) passes quite well, in spite of Stewie's comment "Oh, you'd know it was her..." after Brian expressed concern of not being able to identify her.
