NY State drops surgery requirement to change gender marker on Birth Certificate

I just read that as of June 5, 2014 New York State will no longer require detailed surgical notes to change a gender marker on a birth certificate for those born in New York State. (Please note this does not apply to people born in New York City which issues its own birth certificates).

I haven't been able to nail down the exact procedure yet. Perhaps someone with more internet search skills than me is capable?
