I use a 5" Mr Limpy, and I feel like if someone touched it accidentally through my clothes it would feel fake. Also, this packer is pretty big for a guy my size, so I find it most comfortable to let it run down my thigh. That being said, sometimes if I'm not careful about placement you can see it pretty much. I'm wondering if this is the case with cis guys too? One time I was playing cards with my friends and a couple of them started giggling so I said What? And one said "um, whatya got going on down there?" And when I looked down you could see the outline of my packer's shaft.  Does that happen with cis guys too, or are theirs too squishy to do that? I was wearing kind of thin cargo shorts, but if you understand what I tried to describe would a cis guy's only be visible with an erection or flaccid?

That happens to all guys try wearing basketball shorts and having that happen even though most girls I know like it.

Usually is only with an erection but with thin shorts like you said it's possible for a well endowed man... Unfortunately I was born in a guy body so that's my personal experience.

Some cismen look like they have erections even when they don't, but that's mostly a problem with men who have particularly large organs. Anybody's junk might attract attention with flimsy enough clothing though. I used to know a guy who only wore denim because people would accuse him of having erections sometimes when he didn't, and I knew another guy who wore overalls all the time and said it was because he couldn't help getting erections too often. If people were commenting to me about a crotch bulge I would consider a smaller packer or more concealing pants.

At 5", you have an average sized erection. If I poked it through your pants, I'd expect it to be hard, and finding it squishy would be terrified of it There is a lot of variety in penises. Some are rather firm even when limp, and some are very squishy. I once dated a guy that always looked like he had a boner, even though he didn't.

I think most guys hang it right or left in their briefs, though some position it straight up. It's all a matter of preference and comfort. Again, though: variety. As long as you're presenting in a way that you're comfortable with, don't worry about what people think.
