PhD type doctor in Boston area?

I'm working on finding a therapist with a doctorate to write one of my letters for surgery and I want to find the right person. Partially that's personality but also it's about finding someone with the right experience, someone that doesn't have a history of holding most people back for months and months with little reason and someone with experience writing surgery letters. I'm in the Boston area and I'm looking for a therapist with a PhD so any suggestions you have I would really appreciate.
