I'm not sure I agree with most of this. I definitely agree with the experience factor.
As far as most English speakers are concerned there's Suporn and Chett in Thailand, and a few others no one can remember. Suporn and Chett are also the two that don't do penile inversion, and as far as I know also the two most expensive. You can get much cheaper on Thailand so people are not choosing on price alone. All this does suggest that non penile inversion is a big factor.
Its also possible, and I think likely, that Suporn's reputation is based in large part of his technique producing excellent results, so where would he be without it? It would be difficult at least to distinguish himself from any other surgeon even if he were unusually talented (which I suspect he is).
I'm not sure about the gatekeeping. Most people don't seem to realize just how little there is, and go though the whole rigmarole even if they don't need to.
Personally I'd choose Suporn because I think he produces the best results, and I'd still go if he was double the price, or more. The lack of gatekeeping is admittedly a bonus in my case.
