Painful Dialation

I am 4 weeks post operative with Dr Chett and I have been following his dilation instructions religiously. I didn't spend all that time and money to allow the results to get ruined by improper post-op care. So today was my first day with the #2 dilator and I couldn't get it in. The 0 and 1 are no problem and I've only lost about half an inch of depth since first measurement but when I tried the #2 for the for the first time I finally got the tip past the vestibule after 30 minutes and half a tube of lube; its just too tight; three inches in and it was stretched beyond its limits. I tried a few hours later and had even less positive results. Why?

Perhaps Suporns dynamic dilation technique would help?

I don't know. I'm quite confident now after several attempts clearly something isn't right. That much pain and the effort required for just partial insertion is greater than I can tolerate.

With dynamic you could use the dilatory that work to loosen things up so you could go to the larger one later. Depth is most important anyway, not width.

Due to Suporns technique recovery after SRS is more difficult and he requires a different dilation technique to other surgeons - it needs to be more effective. They refer to the original technique as static dilation. You need to be sufficiently recovered to do dynamic dilation, usually at least two weeks post op with Suporn, and he has to give you the ok first. With any other surgeon I'm not sure how you'd know when it's ok, but best not too early just in case.

Things are improving it seems, I was able to get closer to my original depth after 2.5 hours and 1.5 tubes of lube but certainly not getting any easier. The most pain seems to come from the dilator hitting the prostate and I suppose trying to pushing past it. I feel like this is an awful lot of pain and effort for a part I'm never going to actually use; I'm more concerned with the outward aesthetics and the satisfaction I get looking in the mirror now, but I'm not going to stop dilating and ruin what I spent so much time and money to achieve.

Welcome to the site.

I am sorry you are experiencing a tough time dilating. A little ahead of my experience so far. However, there are a lot of members who have gone through some of what you describe. We also have a lot of members who used Dr Chett. Have you tried contacting Chett's office?

I plan to move this to another spot where I think you may get more traffic and advice. Not a comment on the advice you have gotten so far, just trying to cast the net wider.

I also have a chettgina, and today I started with #4...

And well, it was the same issue as with #2 and #3. From 0 to 1 is a joke, but anything bigger is painful. I just press endure the pain and sooner or later it gets in. Rotating while doing it (screwing yourself) helps a lot in getting it. At the third day on it it's much easier. Active dilation helps a lot, although chett barely mentions it on the website. Getting a copy of Suporn's manual helped a lot.

Today I had been preparing mentally for the insertion pain that would come. I screamed a bit, bit it went in, even if I feel as if somebody had punched me in the intestines and I can't run today. Oddly enough, the second dilation was almost painless. Achieving absolute relaxation, slowly deep breathing abdominally, and my body was literally sucking it in.

PS: Regarding getting stuck at the phrostate... I'm doing what they told me: Bent knees until 4-5 inches are in, and extending my legs before getting to 6-6,5".

PS2: I'm a masochist with high tolerance to pain. Getting 2, 3 and 4 to dull depth took me 5-10 cm the first time.

Thank you everyone, I got the dilation problem sorted. It took quite a bit of doing but I was finally able to adapt and overcome; now it's only painful when the dilator hits the prostate gland.

What did you do?

I just kept working at it using a combination of techniques like twisting, and slow steady pressure to push past the vestibule.
