I've been on IM EV for 3 yrs now and injecting every 5 days. My levels measured on day 3 were around 8,500-9,500 pmol/L and once they were at + 14,000 pmol/L, typical of first trimester pregnancy levels. So, no surprise there and I'm thinking you're probably injecting a lower dose than me. Everything else came back normal and as expected given my high E2 levels. So, my doctors saw no reason to change anything.

My testosterone, in contrast to yours, was much lower, around 0.3-0.1 nmol/L. I find it VERY strange that your levels are so high (well within male range) unless these were measured soon after your implant which can bring up levels but if taken chronically, eventually T levels should drop to castrate levels and stay there.
Also, my prolactin levels were much higher. I'm taking oral micronized P every day instead of the first 12 days.
Hot flushes, by the way, is an indication of too low estrogen or testosterone so also seems odd unless this occurs when you stop progesterone or at certain times, when T drops again. You should discuss these matters with your doctor.
Finally, why 3.5 days? What made you and your doctor decide this was the optimal interval for you?
