Two Stage Breast Implant Question

Yesterday I had my first appointment with a plastic surgeon and was told that in order to achieve the results that I want that I would have to have a two stage implant. They basically put a temporary implant in and keep filling it every so often via a needle inserted near the nipple area to stretch the skin until it reaches the size you want and then they insert the permanent implant. Won't this cause unnecessary ugly scarring since they have to cut me twice and stitch me twice in the same area at the bottom crease? Has anyone had this procedure done? I have another appointment today with a different surgeon, lets see what he recommends... Any input would be greatly appreciated  :)

From what I understand there are 4 different locations they can use for inserting implants. If your worried about scarring I would say choose the arm site for the first stage then you can use the Breast crease for the permanent implant.

I have met with three plastic surgeons.  One thinks two stage is necessary and the other two do not.  I know of no objective study or even side-by-side comparison that would support the idea that two stage is needed or more likely, in what specific cases it is needed.  In terms of my consultations, however, I note that the "two-stager" has been doing breast surgery for longer than the other two combined.  Without data, I wonder if it is more like you're born a Catholic or you're born a Protestant.  The other side is clearly nuts.

Personally, not sure if it is worth it...

rmaddy, I will know a little more after my second consultation today. tomorrow I have a third. The problem with the two stage implants is also the cost, 15K+ as compared to 6K for top of the line gummy bears. The good news is that the doctor is writing a letter to my insurance indicating that I cannot take estrogen because of a recent dvt, therefore my tissue will not expand as it would after a couple years on estrogen so that would make the first stage medically necessary thus the insurance will have to pay, at least for the first stage. The doctor assured me that no one can give me the results I want without a 2 stage. She seemed very sincere and knowledgeable. I'll know more after my next 2 appts.

Ejo,

Depending on how much skin you have and the experience of your surgeon, There is a limit on how big you can go for the initial surgery.

If cost is a concern, I suggest that you just get the largest implant your surgeon can comfortably place in your body, either sub muscular or sub glandular. Some time in the future you can have a redo and in fact, many women have redos because implants do not last forever.

Y'know, I was an A cup at most and they shoved 700CC fun-bags into me using a Keller Funnel...  I'm a 36DDD now.  I mean, you've said twice now "the results I want".... what exactly do you want out of this?

I would like something between a C and D. I realize that these are not the terms doctors use, but I was told that the largest he could fit in a one stage operation would be a 450cc, which is about a B cup. After viewing before and after pictures of mtf patients that had never taken estrogen or with lack of breast tissue like myself, I was quite disappointed. The breasts look like mounds that sit very high near the shoulders and the nipples are very low. I have my third appointment tomorrow, which will be my final doctor before I know what route to take. I wouldn't have minded getting a smaller size for now and going larger in the future but the breasts that I saw just looked horrible. They basically looked like a guy that has been working out a lot with large pecs. There was not much femininity to them.

Hi Ejo;

I have been on HRT now for a little over 4 years and I now have a large B Cup, small C (44C). I recently went to see a surgeon in the United Kingdom and he recommended between 350cc to a maximum of 500cc to end up with quite natural looking breasts. He said 500cc would be the absolute maxim that my current development would manage (in a one stage procedure) but that should give me something like a 44D+ (possibly 44DD). He personally recommended going for something like 450cc to 485cc. Cost was UK pounds 3990.

Well there you go, you said it yourself:  you weren't too keen with this doctor's results.  I implore you to find another doctor.

I am living breathing proof that you don't need a sizable amount of breast tissue to achieve good results... just a good doctor.

To add:  This is a pretty solid representation of how much b00bage I had after about 15 months of HRT, and about what I wound up with after augmentation.  A good doctor can do amazing work!  Don't settle for something you're not impressed with!

Thanks for your input and by the way, you look great! I have my appt. with a third doctor in just a few hours. This doctor is the most experienced working on trans females, so lets see what he has to say. I am driving a couple of hours to see him. Lets hope he's worth it. I'll give you an update when I get back home. Oh, for those of you who aren't aware, I had to stop estrogen due to a dvt in my lower leg so my breast tissue did not have enough time to grow. Since then I've been on tha highest dosage of Finasteride and Spironolactone only. So I have encounter a road block, but that's not going to keep me from driving around it  :)

Great news! I just returned home from the doctor and was told that he could fit 650-700 cc implants in me and have them look feminine and with a centered nipple (without moving nipple). I booked a surgery date for 7/20 and left my deposit. It will be cohesive silicone with 10 year rupture warranty + $2500 cash and lifetime warranty if I want them replaced for larger ones of the same brand in the future. The best news of all is that the entire procedure including follow up appointments is going to cost me around $10,000.00 less than the first doctor and $2500.00 less than the second doctor. I'm glad I made the two hour drive today!

Speaking of costs, in South Florida the going rate for a good surgeon is $6000 to $8000. There are cheaper surgeons, but you get what you pay for.

My cost is $5750.00 including the follow up visits. I think that's reasonable considering that the average price in the U.S is $6000. They will be sub muscular cohesive silicone (gummy bears). The doctor performing the surgery has been chief of plastic and reconstructive surgery at one of the top hospital in the Los Angeles area for 16 years, so I think I'll be in good hands.

Ejo, you seem to have found the dr and can get what you want. "Gummy bears" are a good way to go, natural looking and feeling, long lasting.

Yes, thank you for your confirmation and support. Now if I could just get over the fear of the pain and the risk of the anesthesia I'd be alright. I hear it hurts very bad  :(  As long as doesn't hurt as bad as when I had my gallbladder removed I think I'll be alright. That was the worst pain that I have ever experienced.

That's for single stage though, right?  Good price, honestly, and sounds like the dude is legit.

I have not had a BA but I know many girls who have and they all say the tightness in the chest post op is the worst part.  Several have said that you feel like something is sitting on top of your chest for about a month or so.

As for anesthesia, when ever I have a surgery, I talk to the anesthetist and request light anesthesia or twilight sedation, citing a prolonged recovery time as the reason for requesting minimal anesthesia. I usually do well if the anesthetist gives me about half the calculated dose.
