Orchiectomy without a therapist letter?

Just curious, and trying to find out if the therapist letter for an orchie is required, or left to the discretion of the surgeon, and if it isnt required, which surgeons will do it without.

Also, any good surgeons you'd recommend?

(note: I'm fairly new to all of these hoops we have to jump through; Im just looking to get things sorted out.)

Edit: Ive been reading up on some of the other threads, and getting some learnin' in. It has been quite informative, but I didnt catcht he name of any doctors.

My surgeon required two letters, as he is also an SRS surgeon.  Most doctors don't want to remove health organs.

In Uk I was refused double orchie but the surgeon dug out the undescended left one and left the painful right one!
I have a court claim going through about this.
In UK it is well known that double orchie is an established practice with diagnosed TS when there may be delays or refusal to go for SRS.
