Maybe a good way to go would be to just tell the doctor you believe you have Gender Identity Disorder, and simply ask if he/she would prescribe HRT for you. Most likely, they will decline. At that point, politely ask them for a referral to a gender therapist. Some T-girls have actually gotten their HRT just by asking for it outright. Usually, though, the M.D. will require a referral letter, and a gender counselor is your best bet to write one. That's the route I took and that most of us on here took.
The important thing is, you're out to YOURSELF! At this point, this whole matter may take on a life and momentum of its own. If it does, then there's a darned good chance that you truly are trans and should proceed with transition if that is what you want.
Those of us on here who were diagnosed as trans can basically agree on telling you that GID is like opening Pandora's Box or like a runaway train: Ain't no stoppin' it!
