Informed Consent in Cincinnati area

Does anyone know of anyone near Cincinnati who does informed consent for hormones?  I've been going to a therapist since June, but by this point I've figured that she's probably not going to write me a letter; I know she won't anytime in the near future.  She won't even help out with getting my name changed, or changing my name with my university (which doesn't even require a legal change).  I could understand if I were incredibly stupid, or had some sort of mental illness, but she's just like, "eh, it feels premature, but I can't put a finger on why."  The SOC also say you can live full time for at least three months instead of getting a letter, and this would theoretically work out since I've been full time the whole time I've been in college, but how are you supposed to prove that?  Okay, long explanatory rant over.  If anyone has any advice apart from "therapy really is a good idea, just be patient and wait," it would be appreciated.
