Firstly... just my experience... only have GCS/SRS... unless there are some serious issues that would rule that out.
It is quite enough for our system to go through that on its own. That is a recommendation.
Secondly... accommodation is a money issue and also a convenience/lux issue.
I had decided both times to stay right next door at the Baan Suan for Thai Baht 700 (down stairs, no micro, no kitchen sink...) 2nd time upstairs for Baht 800 with micro and sink.
You will not like much "humping" upstairs with a sore fanny, believe me!
BTW, next block from this place is a BIG-C supermarket, plus plenty hawker stands along the road, as well as one small restaurant even offering "room service". Their food comes recommended – just don't ask for hot, as it will be Thai HOT

Thirdly... you'll be in PHI for two weeks and one more week required for post-op after care i.e. stay in a hotel.
Do NOT forget!!! to ask to get a letter for a wheel chair transport at the airport. I flew with Thai Air Interl. and was well taken care of --- I would have been in far worse shape without the wheel chair. So don't try to be a super-woman.
I had to fly via Bangkok from Jhb, as hardly any long range interl. flights go direct to Phuket.
KL and Hong Kong have direct flights for all I recall.
Lastly... PHI will provide you with driver transport from and to the airport and AS WELL from and to hotel, if you decide to stay in some more up-market place, closer to a beach.
(I didn't miss it, but that's just me).
PHI has a number of OK deals with some of the better hotels and you can check that out on PIH's web-site.
The Baan Suan is so reasonably priced that they do not allow for any PIH discounts at their rates. It is clean, friendly, and quite OK to stay there for the 3rd week of your sojourn. (My take...)
